How Independent RCA Works
A fully certified Apollo RCA Facilitator and instructor will come out to your site and facilitate a comprehensive and thorough incident investigation into the issue at hand.
The facilitation itself can take between half a day and two days to complete (depending on the complexity of the problem) and requires a cross functional team of internal employees who may have been present in the lead up to, and occurrence of the incident.
Prior to the RCA session, your facilitator will familiarise themselves with existing documentation on the failure and failure history. Following the RCA session, they will generate reports and action items out of the session, to share with all stakeholders.
